概要 | This study investigated the applicability of wastewater-based epidemiology (WBE) to carbapenem-resistant Enterobacterales (CRE) using environmental samples collected in Japan. A total of 247 carbapenem-resistant isolates were obtained from wastewater, treated wastewater, and river water. Treated wastewater was identified as an efficient target for CRE monitoring. The isolate analysis indicated that WBE may be applicable to Escherichia coli carrying New Delhi metallo-β-lactamase (NDM)-type carbapenemase, and to the Enterobacter cloacae complex and Klebsiella pneumoniae complex carrying IMP-type carbapenemase. Although several isolates harbored Guiana extended spectrum (GES)-type carbapenemase, their clinical importance remains unclear. Klebsiella aerogenes was rarely detected in environmental samples despite its frequent occurrence in clinical settings. KPC-type, oxacillinase (OXA)-type, and VIM-type carbapenemases were not detected, reflecting their low regional prevalence. These findings suggest both the usefulness and limitations of applying WBE to CRE surveillance. |
